We are thrilled to announce the launch of LifeSpan Advanced Care, CarDon’s new Nurse Practitioner-led program designed to bring greater comfort, consistency, and connection to the residents and staff we serve.
With 10 dedicated Nurse Practitioners — many of whom have grown within the CarDon family — LifeSpan Advanced Care reflects our continued commitment to providing compassionate, coordinated, and resident-centered care.

As the post-acute landscape continues to evolve, CarDon recognized a need for deeper continuity and stronger clinical collaboration within our communities. Too often, care transitions can slow down decisions and create uncertainty for residents, families, and staff.
LifeSpan Advanced Care was created to change that. By embedding Nurse Practitioners directly within our communities, we’re ensuring:
- Faster, more consistent clinical follow-up
- Stronger alignment with our attending physicians and medical directors
- Improved communication and decision-making at bedside
- Greater confidence among staff and families that care remains seamless and personal
This model allows our NPs to truly become part of each community — understanding each resident’s journey, anticipating their needs, and supporting our teams in real time. It’s a model grounded in trust, compassion, and innovation — one that keeps the heart of care close to home.
Our NPs work side-by-side with medical directors to ensure timely attention, improved outcomes, and a more unified approach to care. Currently, four CarDon communities are fully supported by a CarDon NP Monday through Friday, and the results speak for themselves:
- Improved satisfaction for residents and staff
- Stronger partnerships between nursing teams and providers
- Greater comfort and peace of mind for residents and families
