CarDon & Associates, Inc. is very excited to announce our affiliation with Provider Partners Health Plans (HMO-ISNP). Provider Partners Health Plans is a Medicare Advantage Plan that is offered in many our facilities. Provider Partners Health Plans is specifically designed to meet the needs of nursing home residents with Medicare Part A and Part B.
At the center of the Provider Partners Health Plans care is a Nurse Practitioner and RN Care Coordinator who provides an additional level of care that is focused on prevention and early intervention. The Nurse Practitioner works with your primary care doctor to perform physical examinations and create a personalized care plan for your health. They visit you regularly, sometimes daily, and coordinate the care you receive from the nursing home team and other health care providers as well as communicate with family members.
