The Open Enrollment period for the following senior care plans is just around the corner — October 15 – December 7. There are many options including some of the following Special Needs Plans:
I-SNP/IE-SNPs:
- Institutional Special Needs Plans — For eligible individuals who, for 90 days or longer, have had or are expected to need the level of services provided in a skilled nursing facility (SNF).
- Institutional Equivalent Special Needs Plans — For eligible residents living in the community who require institutional level of care, typically in an assisted or independent living community.
- Provides care oversight by Nurse Practitioners and Registered Nurses to support advanced care planning, facilitate communication, and manage changes in condition onsite, avoiding hospitalization when appropriate.
- Common supplemental benefits include hearing, dental, vision, over-the-counter products catalog, and transportation.
D-SNPs:
- Dual Eligible Special Needs Plans — For individuals who are entitled to both Medicare and Medicaid.
- Wrap around benefits, such as grocery, utility and over the counter products, but no care oversight like with I-SNP and IE-SNP plans.
